Hospice of Kitsap County Patient Bill of Rights
Policy
Each patient who receives care from Hospice of Kitsap County is entitled
to the following rights:
- To chose the licensed hospice provider of their choice in Kitsap
County.
- To have informed participation with the hospice professional team
in treatment planning, decision making and management of his/her illness.
- To be informed of the scope and nature of hospice services, qualifications
of the individuals providing in-home services and the procedures for
contacting persons whom are responsible for overall clinical care.
- To receive courteous and respectful treatment, privacy coordinated
services and freedom from abuse and discrimination.
- To refuse hospice treatment or services and to be informed of the
consequences of refusing these services.
- To be accorded confidentiality of personal information and information
contained in the medical record to the extent provided by law or personal
consent.
- To be informed of possible reasons for discharge or transfer to another
agency or level of care, of ongoing care requirements and other available
services and options, if needed.
- To be informed of care provided by hospice, which has experimental
or research aspects and to give written informed voluntary consent to
participate in such programs.
- To receive information regarding billing charges and policies, payment
and insurance coverage, and an itemized monthly billing statement unless
services are reimbursed through a managed care plan.
- To have access to information contained in his/her medical record.

you may call the Executive Director or the Director of Nursing at (360)
698-4611. If you are not satisfied with Hospice of Kitsap County’s
resolution of the complaint or problem, you may contact the Washington
State Department of Health, Monday through Friday from 8:00 a.m. to 5:00
p.m. at 1-800-633-6828. |
